This price list is required by the Federal Trade Commission for the protection of the consumer.

ITEMIZED SELECTIONS
The goods and services shown below are those we can provide to our customers. You
may choose only those items you desire. However, any funeral arrangements you select
will include a charge for our basic services and overhead. If legal or other requirements
mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected.
PROFESSIONAL SERVICES
BASIC SERVICES OF FUNERAL DIRECTOR AND STAFF…............3,150.00
Our Basic services charge includes but is not limited to:
- 24 hour availability of funeral director and/or staff to respond to initial request for services.
- Arrangement conference with family or responsible party, arrangement of service, tribute or memorialization.
- Staff to advise, assist and recommend appropriate personalization and individualization of the services to commemorate the unique life of your loved one.
- Obtaining and recording vital statistics and biographical information.
- Preparation and or transmission of obituary notice.
- Death certificate preparation, filing and obtaining necessary permits, authorizations, consents, and assignments.
- Coordination with other parties such as hospital, doctor’s office and staff, cemetery, crematory, clergy, and others involved in the services and disposition.
- Sheltering and care of decedent. (up to 48 hours)
- Also included in this charge are overhead expenses relative to our facilities such as professional licensing, administrative services, insurance, maintenance, building and utility expenses, taxes, legal and accounting fees, record keeping, equipment and inventory costs.
This fee for our basic services and overhead will be added to the total cost of the funeral arrangements you select. (This fee is already included in our charges for direct cremations, immediate burials, and forwarding or receiving remains.)

Except in certain special cases, EMBALMING IS NOT REQUIRED BY LAW. Embalming may be necessary, however, if you select certain funeral arrangements, such as a funeral with viewing. If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it, such as direct cremation or immediate burial.
This funeral home reserves the right to restrict or refuse the public viewing of unembalmed bodies.
OTHER PREPARATION OF THE BODY

We believe the services of the funeral home are essential for the family to grieve properly. A personalized service provides a time to say goodbye, and - just as important - a time for family and friends to remember and pay tribute to a life that was lived. Everyone leaves a wealth of memories and associations, and it is that legacy that needs to be commemorated.

Visitation at the Funeral Home:
Our services will include set-up of visitation area, use of visitation room, placement of encased remains, display of floral arrangements, parking lot assistance, and supervision and attendance during the visitation.

Our Charge includes: basic services of funeral director and staff; a proportionate share of overhead costs; removal of remains (local); embalming or other preparation of remains, if relevant; local transportation to airport. This charge does not include visitation, rites or ceremonies, or merchandise prior to forwarding the body.

NOTE: Our prices for immediate burials are based on the fact that services are performed at our convenience, and do not allow for the attendance of family, friends or representatives of the deceased. Our charge for a direct cremation without (ceremony) includes: transfer of remains to our funeral home, local transportation to crematory, basic services of funeral director and staff, sanitary care, preparation of remains for identification prior to cremation, up to 48 hour custody, and necessary authorizations. If you want to arrange a direct cremation, you can use an alternative container. Alternative containers encase the body and can be made of materials like fiberboard or composition materials (with or without an outside covering). The containers we provide are corrugated cardboard. Does not include cash advance items.

DIRECT CREMATION OPTIONS
All of our direct cremation options start with the same level of service and include the following:
- Basic Services of funeral director and/or staff.
- Transfer from local place of death to our funeral home.
- Sanitary care (washing, disinfecting and dressing).
- Filing of death certificate and acquiring disposition permits.
- Placing of deceased in the chosen cremation casket or container.
- A viewing for identifying purposes.
- Transfer to crematory.
The selection of a suitable cremation casket or container, the Medical Examiner Fee of $150.00 and the Crematory Fee of $290.00 must be added to the services listed above.
All cash advances will be added to the services you select.
Our minimum container is $229.00, and is suitable for private viewing for identifying purposes by the immediate family only.
For those wishing only this minimal level of service, we offer the following direct cremation options:
BASIC DIRECT CREMATION $4,830.00
Our Basic Direct Cremation is the minimal level of service offered by this funeral home. It is for those requiring only the most minimal services from the funeral home.
STANDARD DIRECT CREMATION $5,945.00
The Standard Direct Cremation incorporates the elements listed above and adds the following additional services:
- Hearse to transport cremated remains to cemetery.
- Licensed Funeral Director and Staff for local graveside service.
TRADITIONAL DIRECT CREMATION $6,145.00
The Traditional Direct Cremation incorporates the elements listed above and adds the following services:
- A memorial Service at the funeral home or local location of the family’s choosing.
- Hearse to transport cremated remains to service location and cemetery.
- Use of vehicle for leading procession/clergy/flowers.
The Traditional Direct Cremation Package is the most complete of all Direct Cremation Packages. The funeral home from the time of death to the burial or release of the cremated remains takes care of all of the necessary and important details surrounding the celebration of the life of your loved one.

PAYMENT POLICY
DuPont Funeral Home, Inc., has established a uniform payment policy to serve all families fairly and to alleviate misunderstandings. This policy enables us to contain our costs to all the families we serve.
“All financial arrangements must be satisfactorily completed before any public notice of funeral service or any merchandise furnished.”
PAYMENT OPTIONS
1. CASH OR PAYMENT BY CHECK: In full at the time of arrangements.
2. ACCOUNTS PAID IN ADVANCE: Through pre-arrangement contract.
3. ASSIGNABLE INSURANCE: Once the assign ability, beneficiary designation, outstanding loans, and any payment deductions have been determined from the insurance company (must be timely verification).
NOTE: Medical Examiner cases may warrant non-acceptance if the manner of death is “Pending” and under investigation by law enforcement (even though the policy is in good standing, assignable, and beneficiary has been verified).
CREDIT CARDS: Accepted credit cards include Visa and Mastercard. May be used to pay part in conjunction with other method or all.
